         Participation Packet Instructions

          Parent(s) and Athlete:

     Participation in conditioning, practices or contests will be denied until ALL forms are
returned completed and with all appropriate parent and athlete signatures and is
reviewed and accepted by the Athletic Department.

     Please return completed forms to the office.  Office hours are Monday - Friday 8:00am - 3:00pm.  Click on link below to access the athletic packet or there are printed copies available outside the guidance office.  If you have any questions, please contact Mrs. Wagner at [email protected]
